🤝 How the Placement Process Works   Â
Initial Inquiry or Referral
The individual, family, legally authorized representative, LIDDA, or HCS service coordinator contacts our team to discuss the need for Host Home/Companion Care. We listen first — learning about goals, concerns, and what a safe, meaningful home environment looks like.Â
 Explore Your Service Options
We discuss available service options based on the individual’s needs, preferences, and support requirements.
If enrolled in HCS:Â Â
We review HCS services, placement options, and compatibility.
If not enrolled in HCS:Â Â
Families are welcome to explore private‑pay and community‑based support options.
Understanding the Individual
We take time to learn about the individual’s routines, personality, goals, preferred location, daily support needs, and what they want from their living environment.
This step ensures the placement reflects dignity, choice, and genuine connection.
Thoughtful Matching
We identify a qualified, dependable host‑home provider whose household and caregiving approach may be compatible with the individual’s needs and preferences.
Meetings and home visits allow everyone involved to determine whether the placement feels appropriate and comfortable.
Approval and Transition
A placement moves forward only after individual choice, required screenings, home reviews, service‑plan coordination, and necessary approvals are completed.
Our team then helps coordinate a safe, organized, and family‑centered transition.
Ongoing Oversight and Support
After placement, we provide ongoing monitoring of health, safety, service delivery, satisfaction, and compatibility.
We work closely with the individual, host‑home provider, family or legally authorized representative, and HCS service coordinator to ensure continued stability and support.
